Reminder you will need to attach a nominator form and permission form if under 18 in PDF format. Please make sure to follow the following format for naming all support materials – YourName-GroupMentorship2024. You will be given the option to finish completing the form at a later date once you’ve started – you will be emailed a link to continue.

When attaching documents, if they are properly uploaded you will see a little red x to the left and (100%) on the right.

Once submitted, if your application has been accepted you will be redirected to a confirmation page. If you do not, you will see an error message and highlights – please check over the whole form as you may have missed a required question or had an attachment error (reminder all files must be PDF).
