Reminder you will need your mentee(s) to submit their form here for your application to be accepted and considered. You will be given the option to save, email yourself a link to complete the application at another time, at the bottom of the page.

Once submitted, if your application has been accepted you will be redirected to a confirmation page. If you do not, you will see an error message and highlights – please check over the whole form as you may have missed a required question or had an attachment error.

A bit about yourself

Give us background on your Artistic Practice. List any professional experience, or mentor experience you have.
Do you know your mentee(s) previously? Tell us how you came to submit for this program with this mentee (or mentees) and why you think this would be a good opportunity for them to work with you.

Program Proposal

Provide a detailed program plan that you and your mentee(s) have agreed upon. Is there a specific project or goal you’d like to achieve with them? Do you have any ideas as to activities you will do together? Ways you will connect them with others in the artistic community? Are there opportunities for them to shadow your artistic work during this time? Be as detailed as possible. (Max 250 words)
Provide some detail on how you plan to connect – in person? Virtually? Is there travel time to consider? Good connectivity in the area? Do both parties they have the equipment and connection needed if meeting virtually?
What are the estimated start and finish dates for your proposal. Remember this should be a 5 month period ending no later than June 1, 2025 – but exceptions can be made if you reach out to the Program Manager before applying (mentor@creativemanitoba.ca)
